The Greens at Pinehurst is seeking an experienced, detail-oriented RN/LPN MDS Coordinator to join our team!

We're looking for a dedicated nursing professional with strong knowledge of the MDS process, care planning, and regulatory compliance who takes pride in ensuring accurate assessments and quality resident care.

• Coordinate and oversee the RAI/MDS assessment process, ensuring accuracy, completeness, and timely submissions

• Monitor completion of MDS assessments, CAAs, and interdisciplinary care plans

• Ensure accurate transmission of MDS assessments and obtain validation reports

• Schedule and facilitate interdisciplinary care plan conferences

• Participate in monthly Quality Measure (QM) meetings

• Manage the care planning process while maintaining regulatory compliance

• Collaborate with nursing leadership to support quality resident outcomes

• Active, unrestricted RN or LPN license in North Carolina

• Graduate of an accredited nursing program

• Minimum 3 years of MDS Coordinator experience in Long-Term Care

• Strong knowledge of MDS assessments, Case-Mix, Medicare and Medicaid reimbursement

• Familiarity with federal and state regulations governing skilled nursing facilities

• Excellent organizational, communication, and critical-thinking skills

• RAC-CT Certification Preferred
