Family Support Partner

Hours:

Flexibility necessary, Weekend hours are rare.

Travel:
Occasional Travel to other UPMC / Children's Hospital Satellite locations

We are specifically looking to hire someone with first-hand experience raising a teenager with Type 1 Diabetes.

The Family Support Partner will work as part of the team to assist youth and their families. The Family Support Partner will provide intensive levels of peer support, information, and teaching to the parents/caregivers to help strengthen their natural support system. The Family Support Partner will support positive outcomes for the youth and family.

Responsibilities:

* Assist the family in linking to natural and community supports

* Assist with crisis stabilization when needed

* Attend all mandatory trainings and become a credentialed Family Support Partner within one year.

* Other non-essential duties as specified by the immediate supervisor

* Provide direct support, information, and teaching to the parents/caregivers as needed

* Seek coaching, supervision and professional input regularly and when needed

* Work closely with other team members to assure a coordinated approach

* Work with and help youth and families to learn skills, achieve their vision, and to become self sufficient

* Work with youth and families to create and maintain an environment conducive with wellness & recovery

Qualifications:

* High School diploma or equivalent required

* Bachelors degree preferred.

* Knowledge of youth and family mental health and substance use resources and service systems preferred but not required within Allegheny and surrounding County,

* Must have direct personal experience raising a child or children with Type 1 Diabetes

* Ability to work with a diverse group of youth, families, clinical and professional staff, and other key stakeholders.

* Ability to engage parents/caregivers raising a child with T1D to identify family strengths without being judgmental, and to integrate these strengths throughout the provision of services.

* Willingness to share personal story and experiences as appropriate.

* Ability to model effective behaviors and appropriate skills.

* Have a good sense of humor, a common-sense approach, be compassionate, flexible, creative, and a good listener.

* Communicates clearly in both and verbal forms.

* Eager to learn, and to be videotaped for credentialing and training purposes.

* Excellent documentation skills.

* Manage time and organization effectively.

* Adapts schedule to meet the needs of the youth, families and team.

Licensure, Certifications, and Clearances:

* Driver's License

* Act 31 / Act 33 / Act 34 / Act 73 with renewal
