Job Title

Post-9/11 Military2VA Case Management Supervisory Social Worker

Duties include, but are not limited to:

• Serves as a Post-9/11 Military2VA Case Management and other VAPHS aligned social work programs supervisory social worker for the VA Pittsburgh Healthcare System.
• Provides oversight of the direct clinical and case management services of 60,500+ Post-9/11 era Veteran registered within the VA Pittsburgh Healthcare System averaging 456 unique Post-9/11 Veterans being registered with the healthcare system monthly
• Completes program oversight for the M2VA, Polytrauma Coordinator, Case Manager(s), Transitional Patient Advocate(s) and Social Work Program Coordinator(s) and other multidisciplinary staff as assigned.
• Provides professional, clinical, and administrative management of their assigned teams and other social work programs as assigned in the Comprehensive Social Services (CSS) Service Line.
• Serves as members of multi-disciplinary teams in a variety of specialty treatment programs in support of post-9/11 era Veterans.
• Facilitates relationships both internally and externally to provide comprehensive services to a highly vulnerable veteran population
• Assists in planning, establishing, and implementing policies and procedures; developing individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating, and overseeing the M2VA staff to which they are assigned.
• Oversees the revision of the local M2VA Program processes and formulate and implement protocols and policies for the program on an on-going basis to meet the ever-changing needs of the program's target population.
