Family Health Physician – High Risk Clinic

Overview

Join our brand-new High Risk Clinic at Berkshire Health Systems in Pittsfield, MA, as a Family Health Physician. This role offers significant autonomy and the chance to help design a program focused on truly comprehensive care for patients facing complex, everyday challenges.

Key Responsibilities

• Conduct in-depth health assessments and develop individualized care plans
• Provide holistic medical care, addressing physical, mental, and social needs
• Coordinate with Behavioral Specialists, Care Coordinators, and nursing staff
• Assist patients with non-medical needs such as securing housing, employment, or transportation
• Maintain patient engagement without volume-based pressure—no set daily patient quota

Qualifications

• Minimum of 3 years of clinical experience, including primary care
• Experience in integrated care or harm reduction models preferred
• Comfort working with high-risk populations (PTSD, severe anxiety, SUD, disabilities)
• Strong collaboration, communication, and problem-solving skills
