SummaryThe Director of Case Management provides strategic leadership and operational oversight for the hospital case management program. This role is responsible for ensuring effective care coordination, discharge planning, and transitions of care. The Director ensures compliance with regulatory requirements, promotes patient‑centered care, and drives performance improvement across the continuum.
Required: Master's degree in Nursing, Social Work or a related field required. Current CT Registered Nurse (RN) license. Minimum of 5 years of progressive leadership experience in hospital case management or care coordination. ACM certification required.
Hours: 40 control hours
Shift: Monday to Friday 8:30 AM-5:00 PM
