As a Home Health RN, you’ll conduct patient assessments in home settings, develop and update personalized care plans, and coordinate with physicians, therapists, and other stakeholders to ensure seamless transitions of care. You’ll administer medications, manage wound care, monitor IV therapies as applicable, and educate patients and families on disease management, safety at home, and adherence to treatment plans. You’ll document thoroughly in electronic health records, track outcomes, and identify opportunities to optimize quality of life.
QualificationsRegistered Nurse (RN) with current state licensure and valid BLS certification. Experience in home health, IV therapy, and/or palliative care preferred but not required.
