Base pay: $49,100 – $67,500 per year. Pay per visit unit: $31.00 – $44.00.

Full‑time schedule: 40 hours per week.

• Provide professional nursing services under the direction of an RN in compliance with state regulations and Center Well policies.
• Deliver skilled nursing interventions, employing a holistic therapeutic approach, teaching, and training as per the plan of care.
• Apply clinical standards to facilitate problem resolution and achieve patient goals.
• Coordinate with RN supervisor on treatment plan changes and accept verbal physician orders as permitted.
• Maintain patient contact and team communication regarding needs and status changes, including discharge planning.
• Maintain highest standards of confidentiality and professional conduct.
• Communicate to ensure availability of supplies and equipment needed for care.
• Prepare and submit necessary documentation; record shift notes.

• Graduate of an accredited Licensed Practical Nursing program.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, reliable transportation.
• Current CPR certification.
• Minimum two years experience as an LPN/LVN in a clinical setting, preferably home health or hospice.
