The Nurse Navigator is the clinical team support role that facilitates care management. The nurse navigator provides care navigation through daily prioritization of patient services and follow up with referral sources, primary care providers, patients, clinicians, and clinical leaders through the management of incoming communication to the agency.

• Two years of community health experience in a Medicare certified agency.
• Demonstrated clinical competency and current knowledge of the principles and practices of home care.
• Demonstrated familiarity with DPH regulatory standards preferred.
• Demonstrated coaching/team building skills required.
• Excellent interpersonal skills and ability to handle conflict required.
• Demonstrated flexibility both in thought process and in time availability.
• Associate's Degree in Nursing required.
• Bachelor's Degree in Nursing preferred.
• RN License - Currently licensed as an RN in Massachusetts.
• CPR.
• Ability to exercise independent judgment.
• Demonstrated interpersonal skills and problem-solving ability.
• Ability to coach, mentor and motivate others.
• Ability to maintain confidentiality.
• Ability to be flexible regarding working hours.
• Knowledge of community resources.
• Must possess a current driving licence and be able to drive in inclement weather.
• Exemplifies Customer Service Standards.
