Our program provides housing support and streamlined travel logistics to help you settle quickly, while the option to rotate through locations across the U.S. delivers exposure to different payer systems, care models, and community resources. Role specifics and benefits

As a Case Management RN, you will assess patient needs at admission, execute comprehensive care plans, coordinate multidisciplinary care, manage utilization review and discharge planning, and monitor post-discharge follow-up to prevent readmissions. You’ll facilitate transitions, arrange community supports, link patients to home health, rehabilitation services, and social resources, and document outcomes with accuracy. You’ll apply risk stratification, leverage evidence-based pathways, and advocate for patients to ensure care plans align with goals while meeting payer requirements.

In addition to clinical responsibilities, you’ll mentor junior staff, share best practices, and pursue ongoing specialty education in case management, geriatrics, palliative care, or chronic disease management. The program offers a multi-week engagement starting December 22, 2025, with opportunities to extend based on facility needs and performance. Compensation features a competitive weekly pay range of $2,164 to $2,296. There is no guaranteed minimum number of hours;

scheduling remains flexible and is determined by assignment requirements. Beneficial extras include a signing bonus, housing assistance, and extension opportunities. You’ll also benefit from 24/7 support from our dedicated travel team, including help with housing, credentialing, site orientation, and on-the-road needs so you can focus on delivering high-quality patient care.
