American Traveler is hiring an experienced RN Case Manager for a teaching hospital setting requiring 3+ years of acute care clinical experience and MA licensure in hand.
Details- Case Management unit at a teaching hospital
- Days shift, 8-hour shifts from 7:00 AM-3:30 PM (40 hrs/week)
- Only five 8-hour shifts accepted; 10/12-hour shifts are not available for this position
- No weekend requirements other than potential holiday coverage
- No on-call requirements
- Active MA RN license required in hand at time of consideration
- Current BLS certification (AHA or ARC)
- 3+ years of clinical experience in acute care settings such as internal medicine, surgery, orthopedics, mother/child, cardiology, or oncology
- 3+ years of clinical experience in Rehab, Home Health, or Long-Term Care settings will also be considered
- Ability to perform comprehensive admission and discharge risk assessments related to quality, financial, payer benefit, and risk management factors
- Knowledge of regulatory and accreditation standards (Joint Commission, DPH, Health Grades) preferred
- CCM or ACM certification preferred
- RN-BC Board Certification in Case Management preferred
- Meditech Expanse experience highly preferred
- Must have own vehicle
- Requires Skills Checklist for RN Case Manager current within 1 year
- Requires one supervisory signed reference from within the past two years of employment
- Responsible for managing healthcare needs and facilitating quality and cost outcomes for all patients across the continuum of care
- Utilizes a multidisciplinary team approach to negotiate, procure, coordinate, and monitor services and resources for patients
- Reviews patients at admission and concurrently throughout hospitalization for appropriate level of care and resource utilization
- Performs transition care planning activities to secure appropriate post-hospital care arrangements
- Participates in performance improvement initiatives
- Strong organizational, analytical, and data management skills required
- Excellent interpersonal, communication, collaboration, and negotiation skills needed with providers, patients, families, insurers, and government agencies
- Ability to work independently and maintain confidentiality required
- Knowledge of local community resources, Medicare, Medicaid, and third-party payer reimbursement required
- Computer literacy required, including ability to manage multiple open applications
- Must work two designated holiday blocks if scheduled:
Thanksgiving through the following weekend, Christmas Eve through Christmas and the following weekend, and New Year's Eve through New Year's Day and the following weekend - All requested time off must be submitted and verified prior to consideration; no more than 5 days of RTO accepted
- Candidates residing within 50 miles of the facility will not be considered
- 8 hours of non-billable orientation required
