Licensed Practical Nurse - Home Health

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

We are seeking a dedicated and skilled Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide high-quality home health care services to patients. The ideal candidate will be responsible for delivering nursing care to the sick and injured while promoting health, preventing illness, and supporting rehabilitation efforts.

Key Responsibilities

• Provide and document nursing care for patients in a home health setting.
• Intervene to promote, maintain, or restore health and prevent illness.
• Collaborate with healthcare providers and community representatives to coordinate comprehensive care.
• Evaluate patients' and families' responses to nursing interventions.
• Function independently under general guidance, adhering to the established protocols and policies.
• Utilize time management and critical thinking skills to deliver effective patient care.
• Demonstrate emotional stability in emergency situations.

Qualifications

• Current Massachusetts LPN license (must be uploaded with application).
• One year of acute nursing care experience is required; hospital-based experience is preferred.
• Experience in home health care is required.
• Certification in Home Health, including proficiency with OASIS, is highly preferred.
• Experience in infusion therapy is a plus.
• Ability to maintain confidential information and communicate clinical data effectively in both verbal and written forms.
• Must possess a valid driver's license and have reliable transportation for travel in various weather conditions.
• Demonstrated interpersonal skills, flexibility in work hours, and a history of dependability in meeting scheduled commitments is essential.
• Knowledge of home care reimbursement and regulatory guidelines is important.

Additional Information

Must be willing to work every third weekend and provide holiday coverage as required. Requests for time off must be specified and verified at the time of application, with a maximum of five days accepted. Availability to work during major holidays is essential.
