Family Partner

Center for Human Development (CHD) is currently seeking Family Partners to join our Community Transition Team.

The Family Partner assists the Community Transition Team with understanding individual family dynamics, strengths, and the support needed to best assist the family in the youth’s transition home. The Parent Partner will also identify and help facilitate connections to needed services for parents and to assist families in the identification of and connection to community-based resources as an alternative or as a supplement to CTT services.

• High School Diploma or equivalent
• The Family Partner should have/had a child or relative who has aged out of DYS.
• Prior court-related involvement with the MA Department of Children and Families or the juvenile justice system may also be considered.
• Candidates must have previous lived experience to support navigation to those served.

Full Time - Must be flexible with schedule requirements, including the ability to work one evening shift a week with occasional weekends, and holidays as program needs arise.
