Service Net is looking for a compassionate and forward‐thinking Case Manager to join our team at Our Friends Shelter. In this role, you will help individuals and families experiencing homelessness build the foundation for long‑term stability. Through direct support, advocacy, and collaboration, you will assist guests in accessing essential resources as they work toward permanent housing.
Key Responsibilities- Conduct comprehensive intakes and assessments for all new shelter guests.
- Support and advocate for individuals and families in shelter or transitional housing.
- Help participants access and follow through with treatment, medical, vocational, housing, and community‑based resources.
- Hold weekly meetings with guests to review service/treatment plans, set goals, make referrals, and monitor progress.
- Foster a positive and supportive residential environment in collaboration with participants and shelter staff.
- Maintain clear, accurate case management and progress notes, as well as other assigned documentation.
- Bilingual candidates (Haitian Creole / French / Spanish) are encouraged to apply.
- High school diploma or equivalent required, with additional training equivalent to two years of college.
- At least one year of experience working with individuals affected by poverty, mental health challenges, and/or addiction.
- Familiarity with local social service agencies and available community resources.
- Valid driver's license and an acceptable motor vehicle record.
