Join the Hartford Health Care Medical Group as a Family Nurse Practitioner or Physician Assistant in Plainfield, CT. You’ll manage your own patient panel while working alongside 2 physicians, 2 APPs, a dedicated MA per provider, and a nursing team of three. Our collaborative primary care practice offers easy work‑life balance and two nearby offices.

Location: 584 Norwich Rd, Plainfield (10369)

Shift Details: Mon, Wed, Fri (Part‑time, weekdays only)

Schedule: No call required

Key Responsibilities:

• Conduct routine physical examinations, formulate medical, educational, and psychosocial plans to treat patients within scope of practice.
• Develop further evaluation plans, including referrals to specialists or additional health team members.
• Provide appropriate treatment within defined protocols and refer out of scope issues to supervising physician.

Qualifications & Requirements:

• Unrestricted State of Connecticut medical license (or eligible).
• APRN (Family or Adult/Geriatric Nurse Practitioner) or PA.
• New graduates welcome to apply.
• Preferred: 2+ years’ experience in primary care or relevant outpatient setting.
