Social Worker - Responsive Support Program

Qualified by Connecticut State Department of Education #71 School Social Worker.
Responsibilities include:
- Provide individual and/or group counseling for students with specific emotional/behavioral challenges.
- Respond to and manage acute situations like student dysregulation, severe mental health crises, or family emergencies.
- Contribute to assessments, support development of support plans (IEPs/504s), and help with transitions for students in special education programs.
- Link families to community resources and support systems for holistic well-being.
- Help design and implement specializing programs and training for students with social-emotional-behavioral needs.
- Maintain accurate records, case files, and reports on student progress and interventions.
- Serves as a liaison between the school, home and community agencies for the purpose of helping students both directly and indirectly with their academic achievement and social and emotional adjustment.
- Compiles the social developmental history and social work notes on students and their families as appropriate.
- Demonstrates oral and written communication skills in an effective and responsible manner.
- Provides consultative services to school personnel and to families of students experiencing social, emotional and/or academic problems.
- Initiates and/or facilitates DCF inquiries, referrals and follow-up.
- Assists school personnel and parents in dealing with attendance concerns.
- Assesses difficulties of referred students, and, when appropriate, conducts individual, group, crisis and/or meetings for students.
- Attends and participates in educational planning meetings for students.
- Participates in personnel development activities which lead to improved job performance.
- Participates on the Child Study Team and accepts Case Management assignments.
- Adheres to the NASW professional code of ethics.
- Is knowledgeable of DSM-IV diagnosis and how they affect student behavior.
- Has a basic knowledge of psychotropic medications and their effects.
- Performs other duties and responsibilities as directed by administration.
- Other duties as assigned by the Principal, Supervisor or Director.
