We maintain state licensure, CHAP accreditation, and Medicare certification, and partner with doctors and discharge planners for easy, well‑managed referrals and consistently competent medical care.
Position OverviewProvides Physical Therapy services to patients as directed by the plan of care in line with the agency’s philosophy, objectives, and policies. Supervises Physical Therapy Assistants and ensures follow‑through of PT procedures.
Responsibilities- Assist the physician in evaluating patients’ level of function, establishing treatment plan and goals.
- Perform initial evaluations within 48 hours of referral and re‑evaluations every 30 days, coordinating plan of care with the physician as needed.
- Prepare clinical and progress notes and required summaries.
- Revise plan of treatment and contact the physician for change in plan of care.
- Advise and consult with patient’s family and other health care staff.
- Coordinate care with agency DON and other staff as appropriate.
- Observe, record, and report any changes in the patient’s condition and update the agency on changes in the patient’s status or schedule changes.
- Supervise the Physical Therapy Assistant.
- Instruct patient and/or family in the use of equipment, safe patient management, and home therapy programs as needed.
- Communicate effectively with patients, caregivers, and families as well as agency staff.
- Provide quality and appropriate care and report issues or concerns to the DON/Administrator.
- Assure that all tasks are performed in a timely manner.
- Check in on Physical Therapy Assistants every 4‑6 patient visits.
- Must be a licensed Physical Therapist in the state of Illinois.
- Must have at least one year of Home Health Care clinical experience.
- Car with required proof of insurance coverage.
- Maintains annual TB test, CPR certification, and other required certifications.
- Able to read and interpret instructions related to patient care.
- Able to effectively perform and prioritize multiple functions or tasks as needed.
- Must be able to travel to prospective patient’s place of residence.
