About the Role:As a Registered Nurse (Geriatrics) at Plainview Healthcare Center, you will be responsible for delivering high-quality, patient-centered care to our senior population. Your expertise will be critical in addressing the unique needs of older adults, promoting independence, and improving their overall well-being. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patient progress, and provide education and support to patients and families.
Responsibilities:- Provide direct patient care, including assessing, planning, implementing, and evaluating patient care plans
- Develop and maintain strong relationships with patients, families, and healthcare team members
- Conduct initial and ongoing assessments of patients' physical, emotional, and social needs
- Develop and implements individualized care plans to meet the unique needs of older adults
- Collaborate with the interdisciplinary team to coordinate patient care and services
- Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed
- Educate patients and families on disease management, medication use, and self-care techniques
