As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APCs, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.

Schedule: Monday
- Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time). Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call.

On-Call: Phone call only, in a rotation for one (1) week at a time. Average call volume -
