Job SummaryAs a Nurse Practitioner, you will assist in overseeing the clinical care and treatment of patients and families, consult with the patient’s attending physician, in consultation with the hospice physician, and provide clinical direction to the Interdisciplinary Group/Team (IDG) to ensure quality patient care.
EssentialEducation and Experience
Must be a graduate of an approved school of professional nursing. Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas.
Responsibilities & Duties- Collaborating with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
- Consulting with and assisting in the IDG development of a patient/family plan of care to ensure quality care is provided.
- Performing and documenting visits to patients/family based on plan of care and patient/family need.
- Interacting with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician.
- Completing face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.
- Providing clinical assessment results to hospice physicians after making face-to-face encounters with patients, as assigned.

Compensation$110 - $125 per visit, plus mileage (rate of $.50 per mile)
