Legacy Health and Rehabilitation at Pleasant Grove provides a "home-like environment" with professional nursing and therapy services for long‑term care and rehabilitation. The Resident Assessment RN focuses exclusively on patient assessment over an extended period, building deeper relationships with residents and families.

• Coordinate the interdisciplinary assessment process for assigned residents.
• Ensure completion of Comprehensive Care Plans in accordance with company, state, and federal guidelines.
• Monitor residents to confirm that individual Care Plans are followed and update as necessary; report variances to the Director of Nursing and the Care Plan Team.

• Registered Nurse (RN) with a valid state license in good standing.
• MDS experience preferred but not required; training will be provided.
• Knowledge of general nursing standards of care and medical practices.
• Excellent analytical, deductive reasoning, and critical thinking skills.
• Computer literacy and proficiency with electronic documentation programs.
