Job SummaryThe Medical Coder reviews, codes, and processes medical, dental, and behavioral health encounters to ensure accurate and compliant documentation, coding, and billing for a Federally Qualified Health Center (FQHC). Responsibilities include assigning ICD‑10, CPT, and HCPCS Level II codes per federal, state, and payer guidelines, resolving denials, supporting reimbursement, and maintaining compliance with Medi‑Cal, Medicare, HRSA, and commercial insurance requirements. The role also involves staff training, process improvements, and collaboration across billing, compliance, and clinical teams.
Qualifications- High school diploma or equivalent;
Associate’s degree in Health Information Technology or related field preferred. - Minimum two years of outpatient medical coding experience, preferably in a community health center or FQHC.
- Current coding certification (CPC, CCA, CCS, RHIT, RHIA).
- Strong knowledge of ICD‑10, CPT, HCPCS Level II, and outpatient coding guidelines.
- Familiarity with FQHC specific coding and billing, including PPS and wrap/PPS add‑on.
- Proficiency reviewing clinical documentation for accuracy and completeness.
- Ability to analyze and resolve coding‑related denials.
- Advanced knowledge of FQHC coding standards and HRSA/UDS reporting.
- Experience with specialty billing for chiropractic, acupuncture, podiatry, cardiology, etc.
- Knowledge of outside entity account reconciliation.
- Proficiency in EPIC EPM/EHR preferred; wisdom dental software knowledge a plus.
- Excellent time‑management, analytical, interpersonal, and communication skills.
- Strong writing, editing, proofreading abilities.
- Professional demeanor, high confidentiality, positive representation.
- Adaptable, proactive, detail‑oriented self‑starter capable of independent and team work.
- Typing ≥35 WPM with minimal errors.
- Good computer skills using Microsoft Office and Axis departmental systems.
- Must be able to use office equipment (copier, fax, etc.).
- Review and assign accurate ICD‑10, CPT, and HCPCS codes for all encounters.
- Ensure compliance with all billing guidelines and FQHC specifications.
- Verify provider documentation and request clarifications when needed.
- Review and correct encounter data before claim submission.
- Collaborate with providers to improve documentation accuracy.
- Analyze and resolve coding‑related denials; submit corrected claims.
- Support billing team with research on payer guidelines and policy updates.
- Maintain proficiency in UDS reporting and quality metrics coding.
- Collaborate with senior management on HRSA, PPS, and documentation standards.
- Conduct internal chart audits and identify training needs.
- Assist in training staff on coding updates and best practices.
- Stay current on coding regulations, payer updates, E/M guidelines, and FQHC requirements.
- Collaborate with CFO and Billing Manager to improve workflows and efficiency.
- Participate in staff meetings, other meetings, and training events.
- Perform other related duties and special projects as assigned.
