- Administer occupational therapy to patients on an intermittent basis in their place of residence, following physician or advanced practice practitioner orders and the plan of care under the Home Health leadership team.
- Evaluate patient functional status (including muscle function, endurance, visual coordination, communication, self-care ability, and work capacity).
- Assess home environments for hazards and barriers to independent living.
- Participate in developing individualized plans of care, including equipment needs, and establish treatment goals for improved function.
- Communicate the plan of care to referring physicians and other organization professionals.
- Teach new skills or retrain patients in daily activities affected by illness or injury in accordance with agency policy.
- Maintain clinical records, progress notes, and report any changes in patient condition to the physician or APN; submit records per agency policy.
- A graduate of an occupational therapy curriculum accredited by the American Medical Association and the American Occupational Therapy Association.
- 1-year clinical experience as an Occupational Therapist preferred.
- Registered with the National Registration Examination of the American Occupational Therapy Association.
- Interpersonal skills and ability to communicate professionally with staff, patients, and caregivers.
- Knowledge of medical terminology; strong problem-solving, organizational, and attention-to-detail skills.
- Strong verbal and written communication abilities; ability to handle multiple priorities, documentation requirements, and deadlines.
- Reliable transportation and valid automobile insurance.
- Ability to sit, stand, bend, lift, and move intermittently; ability to lift at least 35 lbs.
- Home Health experience preferred.
- Familiarity with Home Care Home Base (HCHB).
