Job Title: Per Diem COTA – Weekend Community-Based Rehab
Company: Home Health Care, Inc.
Schedule: Saturdays and/or Sundays (as available)
Pay: $42 - $46 per visit.
Mileage reimbursement between clients!

OverviewThis community-based COTA role offers the chance to provide weekend-only rehab and support to individuals recovering from illness, surgery, or hospitalization—all from the comfort of their home environment.
Duties- Perform prescribed therapy activities and mobility training in the patient’s home.
- Monitor and report changes to the Occupational Therapist.
- Reinforce therapeutic goals and ensure safety in ADL routines.
- Help patients and caregivers understand adaptive equipment and care strategies.
- MN COTA license.
- Ability to work independently.
- Commitment to quality care delivery.
- Detail-oriented documentation.
