Health Care Assistant Band 3
22.5 hours per week. Flexible, part‑time, permanent. Salary: £24,937 – £26,598 pro rata.
Location: South Hams, Devon – Yealm Medical Centre, Ivybridge Medical Practice and other community sites.
Job SummaryDedicated Health Care Assistant (Band
3) supporting patients in their own homes. Working under the direction of Registered Professionals to deliver safe, person‑centred care that promotes independence, dignity and wellbeing.
- Assist with nursing care and holistic assessment of patients’ daily living activities.
- Support clinical interventions under RGN supervision.
- Monitor and record observations to ensure patient safety.
- Maintain accurate, confidential patient records, including incident reporting.
- Provide all aspects of care in accordance with National and organisational policies and care plans.
- Manage and liaise with equipment and supplies, reporting any issues to line manager.
- Support families and carers with information and guidance.
- Respond to changes in patient care situations, including safeguarding and risk management.
- Participate in training, reflective practice, supervision and team meetings.
- Ensure compliance with infection control and other health and safety procedures.
- Use computer for accurate record‑keeping, stock ordering and communication.
- Essential: 5 GCSEs (grades A–C) or NVQ III in Care or equivalent.
- Desirable:
Phlebotomy, ECG, wound care support, experience in community care.
- Essential:
Relevant experience in a nursing care environment. - Desirable:
Teaching skills, palliative care, continence management, community nursing assistant competency.
- Effective communication – written and oral.
- Good interpersonal and multi‑disciplinary working skills.
- Initiative, independence and time‑management.
- Negotiation, organisation and liaison skills.
- Ability to work under pressure and adapt to changing patient needs.
