Clinical Nurse Specialist (Band
6) – Community
Part‑Time, Permanent – 30 Hours per week. Salary £31,248 – £37,627.20 per annum (Full‑Time Equivalent £39,060 – £47,034). This role offers the opportunity to join a dynamic specialist team passionate about palliative and end‑of‑life care.
Responsibilities- Provide specialist palliative care to patients and their families.
- Conduct complex holistic assessments and plan care to meet patients’ practical, psychological, spiritual, and social needs.
- Work autonomously and collaborate closely with primary and secondary care professionals and hospice colleagues.
- Demonstrate strong communication and teamwork skills.
- Improve quality of life and symptom control for patients with life‑limiting conditions.
- Registered Nurse with experience in palliative/end‑of‑life care (essential).
- Experience working within a multidisciplinary team.
- Ability to work autonomously and manage complex holistic assessments.
