As a Nurse (Band 6) at Libertatem Healthcare Group, you will lead the delivery of complex care packages across Cornwall, Dorset and Somerset. Your responsibilities include:

• Develop personalised care plans in collaboration with service users, families and healthcare professionals.
• Conduct holistic nursing assessments and implement changes to improve client outcomes.
• Manage and motivate a regional care delivery team, ensuring high standards and compliance.
• Support and supervise field‑based care workers and nurses, providing regular supervision and competency assessments.
• Report on targets and standards, driving service improvements and business development.

• Develop and monitor individual care plans that are patient‑focused and realistic.
• Guide and implement changes to enhance client and family‑centred outcomes.
• Oversee day‑to‑day operations of the regional care delivery team.
• Provide leadership and support to care workers and nurses, fostering pride in their work.
• Maintain high standards in documentation and recording systems, ensuring statutory compliance.
• Keep your professional skills up to date and undertake necessary training for personal development.

• Experience in community nursing, particularly with spinal injuries, acquired brain injuries and cerebral palsy.
• Comfortable working with both adults and children.
• Hold a professional nursing registration with no restrictions.
• Ability to build effective relationships with case managers, healthcare professionals and multidisciplinary teams.
• Strong commitment to maintaining quality standards and compliance.
• Proficiency in IT skills.
• Full driving licence.
• Willingness to participate in clinical on‑call duties.
