Position: Frailty Advanced Practitioner (ED/SDEC & Virtual Ward)

Bedfordshire Hospitals NHS Foundation Trust invites applications for a Frailty Advanced Practitioner on a secondment/fixed term contract. The post covers the Luton and Dunstable site, working across acute frailty services and the frailty virtual ward.

The Frailty ACP will operate autonomously with a Frailty Consultant, delivering CGA for frail older adults in ED, SDEC and the Frailty Virtual Ward, with rotations to community settings to support the frailty pathway.
