About the Role

Newham is seeking a dedicated and compassionate Social Worker to join the Young Adults Preparation Team (Transitions). This rewarding role focuses on supporting young people with additional needs as they transition from children's services into adult social care, ensuring they receive the right support to achieve independence and positive life outcomes.

Key Responsibilities

As a Social Worker within the Young Adults Preparation Team, you will:

  • Manage a caseload of young people aged 16–25 who are preparing for adulthood and transitioning into adult services.
  • Undertake strengths-based assessments, care planning, and reviews in line with statutory duties.
  • Develop person‑centred transition plans that promote independence, wellbeing, education, employment, and community participation.
  • Work collaboratively with young people, families, schools, colleges, health professionals, and partner agencies to ensure seamless transitions.
  • Assess eligible needs under the Care Act 2014 and support effective transition planning from children's to adult services.
  • Safeguard young adults while promoting choice, control, and independence.
  • Maintain accurate, timely case records and contribute to positive service outcomes.
