Northumberland County Council seeks a qualified Social Worker to join the Home Safe Hospital Discharge Service at Northumbria Specialist Emergency Care Hospital. The role is permanent, 37 hours per week, with a salary of £26k-£37k per annum.
You will assess complex psychosocial needs, develop care plans, deliver interventions to facilitate safe hospital discharges, and collaborate with service users, families, and multi-agency partners. Registration with Social Work England is required.
