Job ResponsibilitiesSummary:
The Case Management Assistant serves as the frontline access point for individuals and families seeking assistance from the Community Action Agency. This position is responsible for triaging clients, identifying immediate and long‑term needs, and connecting them with the most appropriate internal program or external partner resource. The Coordinator plays a key role in the agency’s Comprehensive Intake and Assessment process, ensuring that each client receives person‑centered, equitable, and effective service from first contact through resolution.
The Assistant will perform routine clerical duties to support the Case Management department.
Duties/Responsibilities:
Client Engagement & Intake
- Greet all walk-in and referred clients in a welcoming, respectful, and trauma-informed manner.
- Conduct initial screening and needs assessment to identify barriers to self-sufficiency (housing, utilities, food, employment, transportation, childcare, or health).
- Determine eligibility and route clients to appropriate Case Management, LIHEAP, Weatherization, Head Start, Transportation, or other supportive services programs.
- When appropriate, resolve issues immediately by providing direct information, completing simple applications, or issuing emergency funds.
Case Management & Coordination
- Enter client data accurately into the agency’s case management information system following guidelines.
- Generate and track referrals to ensure follow-through and continuity of care.
- Collaborate with program coordinator and case managers to wraparound services for households with multiple needs.
- Participate in case staffing meetings to discuss client progress, service gaps, and community resources updates.
- Performs clerical duties including typing, filing, and completion of simple forms.
- Operates office machines including copiers, scanners, phone and voicemail systems, personal computers, and other office equipment.
- Answer phones, direct calls to appropriate individuals, and prepare messages.
- Copies, sorts, and files records related to office activities, business transactions, and other matters.
- Prepares letters, memos, forms, and reports according to written or verbal instructions.
- Maintains filing systems either manually or electronically.
- Manages calendars and schedules appointments.
- Intercepts and responds to incoming emails.
- Performs other related duties as assigned.
Program Integration & Community Resource Navigation
- Maintain up-to-date knowledge of community resources and develop working relationships with local service providers.
- Identify trends in client needs and communicate them to leadership to inform community needs assessment and strategic planning.
- Support the agency outreach, intake fairs, or special projects that improve access to services.
Required Skills/Abilities:
- Ability to type at least 45 words per minute.
- Excellent verbal and written communication skills.
- Excellent interpersonal and customer service skills.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Proficient with Microsoft Office Suite or related software.
Education and Experience:
- High school diploma or equivalent required.
- Clerical experience preferred.
Monday – Friday 8:00AM – 4:30PM
