A healthcare provider serving rural America is seeking a dedicated Health Navigator to develop trusted relationships with patients and assist with various tasks that streamline care delivery. You will work within partner clinics, engaging with patients, entering data, and facilitating communication. The ideal candidate will be a self-starter with a strong service mentality, an eagerness for problem-solving, and a GED or High School Diploma.

The position offers a full-time schedule from Monday to Friday, 8 AM to 5 PM.
