About the Role

Our client is looking for a dedicated and organized Case Manager to join their Social Services department in Polokwane. This pivotal role involves assessing client needs, developing comprehensive service plans, and coordinating resources to support individuals and families facing complex challenges. You will act as a primary point of contact, ensuring clients receive timely and appropriate support to achieve their goals related to housing, employment, healthcare, and overall well-being.

Your expertise will be crucial in navigating social support systems and advocating for client needs within the Limpopo province.

Key Responsibilities
  • Conduct comprehensive client assessments to identify needs, strengths, and barriers.
  • Develop individualized case plans in collaboration with clients, setting clear goals and objectives.
  • Coordinate and facilitate access to essential services, including healthcare, housing, education, and employment support.
  • Monitor client progress regularly, documenting interventions and outcomes in case files.
  • Provide ongoing support, counseling, and advocacy for clients throughout their engagement with services.
  • Liaise with external agencies, community organizations, and government departments to ensure integrated service delivery.
Requirements
  • A relevant Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
  • Minimum of 2-3 years of experience in case management or social work.
  • Strong understanding of social welfare systems, community resources, and relevant legislation.
  • Excellent assessment, planning, intervention, and evaluation skills.
  • Proficiency in maintaining accurate case records and preparing reports.
  • Strong interpersonal and communication skills, with the ability to build rapport and trust with diverse clients.
