Case Manager

Our client is seeking a results-oriented Case Manager to join their community services team in Polokwane . This hybrid role involves managing client caseloads, assessing needs, and coordinating services to ensure positive outcomes for individuals and families. You will work both remotely, managing documentation and client communication, and in the field, providing direct support and connecting clients with resources. The ideal candidate possesses strong organizational skills, a deep understanding of local social services, and a commitment to empowering clients towards self-sufficiency.

This role is essential for providing crucial support within the Limpopo province.

• Assess client needs, develop individualized case plans, and monitor progress towards goals.
• Coordinate services with various agencies, including healthcare providers, educational institutions, and government support programs.
• Provide direct support and counseling to clients, addressing social, emotional, and practical challenges.
• Maintain accurate and comprehensive client records, ensuring confidentiality and compliance.
• Facilitate client access to community resources and advocate on their behalf.
• Conduct regular case reviews and participate in team meetings, both virtual and in-person.

• Degree in Social Work, Psychology, Sociology, or a related field.
• Proven experience as a Case Manager or in a similar social service role.
• Strong knowledge of social welfare systems and community resources in the Polokwane area.
• Excellent assessment, planning, and intervention skills.
• Proficiency in case management software and virtual communication tools.
