Disability Support Facilitator

Our client is seeking a dedicated Disability Support Facilitator to provide essential assistance and empowerment to individuals with disabilities in the Polokwane area. This role focuses on enabling clients to live more independently and participate fully in their communities by providing tailored support. You will work collaboratively with clients, their families, and other professionals to develop and implement personalized support plans.

• Provide direct support to individuals with disabilities, assisting with daily living, personal care, and community participation.
• Develop and implement individualized support plans based on client needs and goals.
• Facilitate skill development and independence by encouraging clients to engage in meaningful activities.
• Advocate for clients' rights and needs, ensuring access to community resources and services.
• Maintain accurate records of client progress, activities, and any reported concerns.

• Relevant qualification or certification in disability support, social work, or a related field.
• Previous experience working with individuals with various disabilities.
• Strong understanding of disability rights and inclusive practices.
• Excellent communication, empathy, and problem-solving skills.
• Ability to work both in the field within Polokwane and remotely as required by the hybrid model.
