Housing Manager in Pomona, CA oversees programs connecting Medi‑Cal members with housing support, manages teams, and coordinates with health plans and community groups to provide crucial housing help (finding, deposits, maintaining tenancies) as part of California's major healthcare initiative, CalAIM, to address homelessness and health issues. They bridge healthcare and housing by ensuring services address social needs and reduce costly hospital visits for vulnerable people.
Key Responsibilities- Program Oversight:
Develops, implements, and manages housing‑focused programs within CalAIM. - Operational Management:
Manages operations of the Transitional Rent and Housing Trio Programs (Housing Transition Navigation Services, Housing Deposits, and Housing Tenancy and Sustaining Services), ensuring high‑quality service delivery and coordination across all roles. - Staff Supervision:
Recruiting, training, managing, and providing ongoing support to housing navigators. - Client Support:
Ensures staff help members find housing (Housing Transition & Navigation), cover move‑in costs (Housing Deposits), and stay housed (Housing Tenancy & Sustaining Services). - Client Services:
Effectively deliver services, manage caseloads (often with housing navigators), address client needs (health, housing, benefits), and provide crisis response. - Budget & Reporting:
Manage program budgets, oversee data/record keeping, and prepare reports. - Collaboration:
Work with Managed Care Plans, counties, and community organizations to integrate care. - Compliance:
Ensure documentation and services meet program guidelines and quality standards. - Housing Assistance:
Help members find housing, complete applications, secure deposits, and manage move‑ins. - Support & Navigation:
Provide ongoing support for lease compliance, resolve disputes, and build life skills. - Coordination:
Work with doctors, mental health providers, and other community partners to ensure holistic care. - Program Implementation:
Oversee the delivery of CalAIM's Housing Community Supports (e.g., Housing Transition & Sustaining Services). - Strategic Planning:
Monitor program effectiveness, identify improvements, and stay updated on CalAIM changes.
Skills & Qualifications
- Supervisory/Management experience.
- Strong understanding of housing stability, homelessness, and supportive services.
- Knowledge of Medi‑Cal, DHCS, and social service systems.
- Trauma‑informed care, crisis intervention, and active listening.
- Administrative skills, data management, and compliance focus.
- Housing Stability:
Connect clients to resources for permanent housing and prevent homelessness. - Holistic Support:
Address medical, behavioral, social, and employment needs. - Data & Compliance:
Maintain accurate records and adhere to strict program guidelines.
