Prototypes Recovery Bridge Housing (RBH), a program of Health

RIGHT 360, provides interim housing to unhoused adult and adult perinatal women receiving outpatient treatment services. RBH offers recovery-oriented services including peer support, group meetings, case management, and life skills development. It aims to stabilize patients' lives and prevent relapse by offering safe, supportive housing. Residents, prioritized based on vulnerability, receive approvals for 3-month increments up to 6 months annually and must remain engaged in outpatient treatment throughout their stay.

The Specialized Case Manager supports our clients enrolled in housing by assisting them with building daily life skills, budgeting skills, and other housing related skillsets to prepare for transition to permanent housing.

KEY RESPONSIBILITIES Direct Service:
  • Conduct a needs assessment that includes basic physical and social needs.
  • Identify goals and document Individual Service Plans.
  • Provide follow-up to ensure clients are moving toward goals, as per Service Plan.
  • Provide one-on-one Case Management sessions at minimum 1x per week.
  • Support clients when dealing with interpersonal conflicts with roommates/others.
  • Conduct/participate in House Meetings to support clients and promote a safe and amicable environment in the shelter.
  • Encourage clients to be engaged in program and program demands.
  • Support and guide clients in placing work orders when needed for Property Manager.
  • Help develop transition plans, including assistance with housing, aftercare services, etc.
  • Provide community referrals based on individual needs/requests.
Documentation:
  • Utilize electronic record systems as per program and contract requirements.
  • Document one-on-one case management sessions with assigned clients in a timely manner.
  • Create Service Plan as required by contract.
  • Create/guide and support the creation of Behavioral Support Plans for clients with behavioral problems.
  • Document communications with internal and external agencies to ensure consistency in services.
  • Participate in program audit preparations.
Administrative:
  • Support and coordinate with client’s external services/agencies.
  • Support housing navigation process.
  • Coordinate case conferences with external and internal agencies associated with caseload.
  • Inform external and internal agencies/programs about client’s progress/lack of.
  • Participate in staff meetings, team meetings and in-service training.
  • Complete assigned training in a timely manner.
QUALIFICATIONS Education, Certification, or Licensure
  • Bachelor’s degree in related field.
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Valid California Driver’s License and access to registered and insured transportation.
Experience
  • At least two years’ work experience in a supported housing environment.
  • Experience working with individuals dealing with substance use disorders, who are chronically homeless, and/or reentry populations.
  • Experience in providing one-on-one case management sessions and identifying and documenting clear, achievable goals within individual service plans.
  • We will consider for employment qualified applicants with arrest and conviction records.
  • Must complete a background check and livescan.
