Position Title

LTSS Service Care Manager

Residing within the cities of Florida:
Lauderdale, Deerfield Beach, Pompano Beach, Hollywood, or Hallandale

11 months (Contract to Hire)

Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long‑term care members and educates members and their families/caregivers on services and benefits available to meet member needs.

• Required: Bachelor's degree and 2–4 years of related experience

• Required: Valid driver’s license
• Preferred: —

• 2+ years of Care Management experience (field experience is a must)
• Home Health Experience
• Long Term Care Medicaid experience
• Medicaid / Medicare experience
• Ability to manage high case loads – caseloads of 50, 60, or 70 members aged 65 and above (pediatric experience not required)
• Experience with electronic medical health records

• Field-based experience managing a caseload similar to the one noted for this team (60–120 members)
• Previous DFPS experience
• Strong case management performance indicators

• Telephonic customer service calls experience is not a fit for field case management
• No field experience
• No prior experience with high caseloads
• Typing speed less than 40 WPM on a typing test

• Care Management with field experience (2+ years)
• Home Health Experience
• Experience with electronic medical health records
