Overview
Atlantic Health is seeking a Diabetes Nurse Practitioner in Pompton Plains, New Jersey. The NP will support Endocrine Services, outpatient Pompton Plains and inpatient Chilton Medical Center. As a Diabetes NP, you will deliver specialized diabetes care to adult patients 18 and older, develop and implement individualized treatment plans, educate patients, support self-management, and participate in quality improvement initiatives while pursuing ongoing professional development.
Responsibilities- Provide comprehensive diabetes care to adult patients 18 years and older.
- Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.
- Educate patients and support self-management of their condition(s).
- Contribute to growth and advancement of the Endocrine program through quality improvement initiatives.
- Engage in continuous learning to stay updated with the latest advancements in the field.
- Graduated from an accredited NP program
- Master’s degree required
- Certified Diabetes Counselor Education Specialist (CDCES), or ability to obtain certification within 1 year of hire
- ACLS and BLS certification required
- 2-3 years of clinical experience preferred, ideally in an acute care or physician practice setting managing patients with diabetes; new graduates will be considered
- Excellent communication and computer skills
- Strong work ethic and a team player who can work independently
