Advanced Clinical Pharmacist

We have an exciting opportunity for an enthusiastic and patient focused pharmacist to join our team to lead on Palliative Care and pain services.

• Provide a clinical pharmacy service to our local hospices in Wakefield and Pontefract, as well as working with the palliative care and pain teams within the acute Trust.
• Perform clinical duties such as medicines reconciliation, patient counselling and clinical review of medicines prescribed.
• Work with the MDT to provide patient centred pharmaceutical care.
• Provide education and training to clinical staff regarding medicines use in end of life and pain management.
• Lead the development, review and implementation of trust-wide evidence based policies and guidance relating to the specialist area.

This part time (25 hours) role will suit an experienced pharmacist with a strong clinical background and excellent communication skills. You will be adaptable and confident working within a multi-disciplinary team.
