Duties and Responsibilities:
Clinical Role
To assess plan, implement and evaluate patient centered nursing care as per Hospice policies and protocols.
Actively work in partnership with patients and their families and carers.
Ensure patient, family and carers are central to care delivery and decision making.
To always uphold and promote patient safety.
To promote and support Advance Care Planning.
To seek feedback from patients, families and carers and share with the Incare leadership team where indicated.
To communicate effectively with patients, families and carers, the MDT and all staff within the Hospice to promote effective teamwork.
To deliver concise and accurate patient presentation in handovers and to the MDT meetings.
Communicate with other professionals outside the Hospice to facilitate continuity of care.
Ensure accurate and timely records of care (electronic and paper) are maintained, upholding information governance.
Promote evidence-based practice through membership of clinical groups.
Actively promote hand hygiene and infection control policies to minimise infection risk.
Participate in audit and evaluation of clinical practice in the interests of promoting quality care.
Actively promote reflective practice.
Actively promote an environment which enables the nursing team to work effectively.
Management Responsibilities
Prioritise, organise and delegate workload.
Develop skills and undertake training to take charge of the Hospice out of hours, liaising with other professionals, community patients and families.
Participate in attending team/ward meetings.
Act as ward coordinator ensuring the running of the ward is safe, effective and cohesive.
To support the Deputy Nurse Manager ensuring effective staffing levels are maintained to meet patient’s needs, using bank staff to cover unplanned absence.
To be aware of budgetary implications when delivering nursing care.
To be able to manage compliments and complaints in the absence of the ward leadership team.
Educational Training and Development
To keep abreast of current developments, advances and research in palliative care.
To develop knowledge and skills in line with agreed competency frameworks.
Undertake continuous professional development seeking opportunities to enhance skills and to participate as required in approved education programmes.
To participate in mentorship to nursing students and/or other professionals on placement within the Hospice.
Contribute to the promotion of a positive learning environment within the clinical area.
A Preceptorship programme is available (for newly qualified nurses).
General Duties and Responsibilities
All employees are required to abide by the Health and Safety at Work Act, attend annual mandatory training sessions and ensure they always comply with Hospice policies and procedures.
Employees must demonstrate commitment to their own personal development and are required to make a positive contribution to fundraising and raising the profile of the Hospice.
Strict confidentiality applying to all aspects of Hospice business must be observed at all times.
The tasks and responsibilities outlined in this job description is not exhaustive and the post holder may be required to undertake other tasks in agreement with their line manager.
