Become a leader of our in-house therapy program at Goldwater Care Pontiac! As the Director of Rehabilitation (DOR) at our 90-bed skilled nursing facility, you will be responsible for the day-to-day operations of the therapy department, with a focus on achieving the highest level of functional outcomes and resident satisfaction. As the facility's lead therapy representative, the DOR plays an important role on the interdisciplinary leadership team and works closely with staff, residents, families, and other members of the care team to support Goldwater Care's mission and provide quality resident care.

Essential Duties and Responsibilities:
  • Lead a therapy team providing quality physical, occupational, and speech therapy services to approximately 15–20 residents.

  • Approximately 50–75% of the caseload consists of short-term rehabilitation residents working toward safely returning home.

Day-to-day responsibilities include:
  • Develop and maintain a positive and supportive work environment that allows therapy staff to provide quality resident care.

  • Project treatment minutes and develop therapy schedules based on each resident’s individualized clinical needs.

  • Plan and coordinate staffing coverage for PTO, changes in census, and other department needs.

  • Provide insurance updates and manage Part B therapy authorizations.

  • Identify and problem-solve barriers that may affect the delivery of therapy services.

  • Communicate with residents, families, and team members in a professional, compassionate, and respectful manner.

  • Actively participate in interdisciplinary meetings, including:

    • Morning meetings

    • Utilization Review meetings

    • Resident care plan meetings

  • Collaborate with members of the interdisciplinary team (IDT) to provide the highest level of care and support positive resident outcomes.

  • Support the overall operations and performance of the therapy department while maintaining a positive team environment.

  • Maintain a partial clinical caseload as department needs require.

  • The long-term expectation for this position is an approximately 50/50 balance between direct treatment and administrative/department responsibilities
    , depending on census and the needs of the therapy department.

Qualifications:
  • Current and active Illinois licensure as a therapist (
    PT, PTA, OT, COTA, or ST
    ) in good standing.

  • Current CPR certification
    .

  • Previous experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred.

  • Previous therapy leadership or supervisory experience preferred.

  • Strong communication, organizational, and leadership skills.

  • Ability to work effectively with residents, families, therapy staff, and the interdisciplinary team.

  • Ability to balance clinical responsibilities with administrative and department management duties.

Physical Demands:
  • Must be able to stand and walk for extended periods throughout the workday.

  • Must be able to frequently bend, stoop, kneel, reach, push, pull, and perform other movements required to provide resident care.

  • Must be able to assist residents with transfers, repositioning, ambulation, and mobility activities using proper body mechanics and approved equipment.

  • Must be able to lift and/or move therapy equipment and supplies as needed.

  • Must have the physical ability to provide hands-on assistance to residents with varying levels of mobility and physical ability.

  • Must be able to respond quickly and safely to residents who may require assistance.

  • Must have sufficient visual, hearing, and communication abilities to safely perform job duties and monitor residents during treatment.

  • May be required to work in various areas of the facility, including resident rooms, therapy areas, hallways, and common areas.

  • Must be able to tolerate frequent interaction with residents and staff and work in an environment with varying levels of noise and activity.
