Home Provider​/Shared Provider

As a Home Provider, you will open your home to support individuals with intellectual disabilities, fostering a sense of belonging, safety, and independence.

• Support daily living skills such as cooking, cleaning, budgeting, and decision‑making.
• Assist with medication management, medical appointments, and personal care activities.
• Help individuals build community connections, participate in activities, and pursue personal goals.
• Provide transportation and support daily routines as needed.
• Maintain documentation and communicate with the support team.
• Offer behavioral, medical, or physical support depending on individual needs.

• Must be at least 21 years old and a resident of Illinois.
• Own or rent a home, with active insurance.
• Reliable transportation, valid driver’s license, and vehicle insurance.
• Internet access and basic computer skills.
• Ability to complete screenings, background checks, and training.
• Registered healthcare worker (DSP) preferred.

• Ongoing training and dedicated team support.
• Resources for licensing and program compliance.
• Average annual income of approximately $33,000 (tax‑free) plus household expense contributions.
