Neuro Stroke Coordinator - RN | Saint Joseph Mercy Health System

Purpose

Assists in the management of the Stroke Program as an expanded role with increased responsibility through the coordination and oversight of stroke program services within the Southeast Michigan Market, working with others to implement and sustain medical, service line, and inter‑departmental components.

Education & License Requirements
  • Baccalaureate of Science in Nursing (BSN) degree from an accredited school of nursing required.
  • License as Registered Nurse for State of MI (RN) required.
  • Master of Science in Nursing (MSN) preferred.
  • Stroke related certification preferred.
Functional Role
  • Coordinates services to achieve program goals and metrics; interacts with patients, colleagues, physicians, families, and the community fostering a safe, collaborative environment.
  • Assists in operations, quality of care, and program evaluation; develops protocols, policies, and procedures; supports all age groups; captures, analyzes, and reports data to leadership; conducts audits, produces reports, and recommends improvements; coordinates improvement plans; provides expertise and support in accreditation and regulatory processes; manages certification submissions.
  • Demonstrates commitment to safe practices and positive enhancement of patient and colleague experiences; acts as a resource and liaison.
Assignment & Responsibilities (not all inclusive)
  • Serve as a resource for stroke patients and families.
  • Maintain timely communication and responses to patients.
  • Track patients treated for suspected or confirmed stroke/neurovascular events; confirm post‑stroke appointments and follow‑up plans.
  • Maintain communication and collaboration with key contacts, clinics, physicians, and EMS agencies; partner with stroke physicians for coordinated care.
  • Establish workflow with fellow coordinators; identify and resolve process improvement actions; set task expectations; record time spent on coordination activities.
