Neuro Stroke Coordinator – RN

Purpose

Assists in the management of the Stroke Program as an expanded role with increased responsibility through the coordination & oversight of stroke program services within the Southeast Michigan (SEMI) Market. Works with others to implement & sustain medical, service line, and inter‑departmental components of the program.

Employment Type

Full time – Day Shift

Education & License Requirements
  • Baccalaureate of Science in Nursing (BSN) from an accredited school of nursing – required.
  • Registered Nurse license for the State of MI (RN) – required.
  • Master of Science in Nursing (MSN) – preferred.
  • Stroke‑related certification – preferred.
Functional Role
  • Coordinates services to achieve established program goals and metrics; interacts with patients, colleagues, physicians, and community members to promote a safe and collaborative environment.
  • Assists in operations, quality of care, program evaluation, and development; develops and maintains program foundations (protocols, policies, procedures); supports care practices for all assigned age groups.
  • Provides data capture, analysis, and reporting to assist leadership in achieving high‑quality care, operational efficiency, and regulatory alignment; conducts audits and suggests improvements to processes.
  • Supports accreditation or survey processes, coordinates certification submissions, and maintains registry relationships and data.
  • Acts as a resource and liaison for patients, families, and staff.
Assignment & Responsibilities (not all-inclusive)
  • Serve as a resource for stroke patients and families at the assigned location.
  • Maintain timely communication and responses to patients.
  • Track patients treated for suspected or confirmed stroke/neurovascular events.
  • Confirm post‑stroke patients have appointments and follow‑up plans.
  • Maintain communication and collaboration with stroke/neuroscience contacts, clinics, physicians, and EMS agencies.
  • Partner with stroke physicians for a coordinated team approach.
  • Establish workflow with fellow Stroke Coordinators; identify and resolve areas for improvement; set task expectations and record time spent on stroke coordination activities.
