Main duties of the jobThe Care Navigator willalso be expected to undertake reception duties as part of the role, supporting the administrative team in delivering a polite and professional service to the entitled patient population.
About usThis long established practice can trace its roots to the turn of the century. The modern practice originates from the mid 20's and the title "The Mount" is taken from the first purpose-built surgery erected in the early 30's.
In March 1986, The Mount Surgery moved to George Street and absorbed the old Pontnewynydd branch.
The new Mount Surgery is housed in Pontypool Medical Centre, off Town Bridge. It opened in June 2002 and is a modern, spacious, fully computerised practice. With six partners and numerous practice and attached staff, we offer a comprehensive range of primary health care.
New Inn was originally a small surgery attached to the local doctor's house moving to its local Greenfield site in 1975, now rebuilt as the purposeful and attractive New Inn Medical Centre which opened in 1993.
Job responsibilities- Opening up/locking-up of Practice premises and maintaining security in accordance with Practice protocols
- Reception counter
- Maintaining and monitoring the Practice appointments system
- Processing personal and telephone requests for appointments, visits and telephone consultations and ensuring callers are directed to the appropriate healthcare professional
- Processing and distributing incoming (and outgoing) mail
- Taking messages and passing on information
- Filing and retrieving paperwork
- Processing repeat prescriptions in accordance with Practice guidelines
- Computer data entry/data allocation and collation; processing and recording information in accordance with Practice procedures
- Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers
- Providing clerical assistance to Practice and Trust staff as required from time to time, including word/data processing, filing, photocopying and scanning
- Ordering, re-ordering and monitoring of stationery and other supplies
- Provision of refreshments for staff and visitors as required; loading and emptying the dishwasher and keeping the kitchen area clean and tidy
- Keeping the reception area, notice-boards and leaflet dispensers tidy and free from obstructions and clutter
- Health & Safety
- Administering the DNA policy for non attendance of appointments at hospital.
- Workflow optimiser downloading hospital/clinical letters, reading and directing as necessary, coding into patients notes.
- Clear and professional telephone manner
- Experience working in Primary care
