Social Worker, Mental Health

As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments,facilitate group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future.

You report to the Safeguarding Lead, and will manage internal investigations and liaise with home authorities for service users during their admission and discharging process, while enjoying access to arange of training opportunities that can see your career grow and you achieve your career aspirations.

As a Social Worker you will be:

• Referrals to LADO and Local Authority in line with safeguarding and assessments
• Educating Hospital staff about relevant safeguarding matters.
• Completing a full annual social care reassessment within the CPA format
• Facilitating access to welfare rights, housing, advocacy services and associated support services
• Participating in 1:1 supervision

To be successful in this role, you'll need:

• Registered with Social Work England or applied for
• Experience with in similar environment
• Experience with working with vulnerable adults

You will be working at Aderyn, ahigh quality step downservice for men with enduring mental health conditions sets a sector-leading example of clear care pathways for service userwith a focus on recovery.

As well as a17 bedrehabilitation unit there is also a2 bedopen rehabilitation cottage.

The unit is situated in a picturesque part of Wales with good transport links.

You willworkingalongside a multidisciplinary team to provide service users with opportunities to improve their independence and prepare for an independent life in the community. Individual and collaborative therapeutic timetables are developed for each patient and individual psychological therapies are offered.
