Pembrokeshire College is seeking a Learner Transition Co-ordinator to support the successful transition of learners with Additional Learning Needs within the Inclusion Team. The role involves liaising with external agencies and coordinating person-centred meetings, with a strong emphasis on organisation and communication.
The post is a full-time permanent position based on a 37-hour week, with a 4-point incremental pay scale (£29,440–£31,878 pro rata) and 28 days holiday per year (pro rata) plus
