Practice Nurse - Parc Canol Group Practice

We are looking for a motivated and friendly person to provide a first class nursing service to practice patients, working closely with healthcare colleagues and in collaboration with the GPs.

Part-time hours may be considered for the right candidate.

Job responsibilities
  • Practice independently in accordance with RCN code of conduct and practice guidelines, referring appropriately to GPs.
  • Maintain accurate and complete documentation of patient care.
  • Ensure continuation of personal and professional development.
  • Undertake patient nursing services including chronic disease monitoring consultations, wound treatments (applying and removing dressings and sutures), administering immunisations and vaccinations (including children), providing contraceptive advice, health education including travel advice, taking samples, swabs and specimens, checking vitals, carrying out physical examinations, investigatory procedures, and cervical smears.
  • Participate in the annual appraisal system for the practice nurse team.
  • Share responsibility for effective intra‑team and practice‑wide communication.
  • Participate in regular nursing/healthcare team meetings.
  • Provide professional advice on practice developments and recommend changes to working practices in line with evidence‑based knowledge.
  • Assist in promoting and maintaining health, safety and security, including infection control, risk identification, training participation and reporting hazards.
  • Support equality, diversity and rights of patients, carers and colleagues, and adhere to relevant policies and legislation.
  • Maintain quality through alerting team members to quality issues, assessing own performance and contributing to team effectiveness.
  • Communicate effectively with team members, patients and carers, recognising their needs for alternative communication methods.
  • Apply practice policies, standards and guidelines, and discuss their impact with the team.
Person Specification
  • Registered Nurse (NMC) with nursing degree or equivalent experience.
  • Evidence of immunisation training and competence assessment within the last 2 years.
  • Postgraduate diploma or degree in primary care (desirable).
  • Experience of chronic disease management.
  • Experience of managing anti‑coagulation (desired).
