Discover an extraordinary opportunity as a Social Worker in our Children with Disabilities Team. This role gives you the chance to work collaboratively to provide crucial support to children with a lifelong, permanent, and substantial disability. Based in Pontypridd, our team comprises five specialist groups, including the Enablement Team, 0-11 Team, 11+ Team, Eligibility Team, and Occupational Therapy Team. Join us in delivering early intervention, working hand in hand with families and agencies like health and education.

Your primary role will involve conducting eligibility assessments for continuous support and intervention. Importantly, this is not a case-holding role.

What you will do
  • Conduct thorough eligibility assessments for children requiring continual support from the Disabled Children's Service.
  • Collaborate effectively with a multidisciplinary team, including agencies involved in health and education.
  • Engage with children and their families to ensure a holistic approach to early intervention and support.
  • Provide expert guidance and input into care plans to improve outcomes for children with disabilities.
  • Attend regular team meetings to share insights and develop strategies for continuous service improvement.
