Supporting people with complex clinical care needs, autism, learning disabilities and mental health
About the roleProvide and coordinate individualised care and resources to service users living in their own homes and within the community. You will manage and distribute resources effectively and will ensure that all care is provided in a respectful way; keeping in line with all relevant policy and procedure. You will work closely with the multidisciplinary teams, such as Social Workers, District Nurses, Clinical Commissioning Groups and Service User family members.
You will ensure the effective management of these complex relationships and with the support of your branch manager will deliver the care and wellbeing service to the individuals being supported.
- Oversee all non clinical aspects of clients care packages (updating care plans and risk assessments, completing monthly reviews etc)
- Build and maintain professional relationships with clients and carers based on trust, respect and honesty
- Complete carer supervisions, appraisals and team meetings
- Liaise with the rota coordinator and others to ensure all shifts are filled and plan in advance for any annual leave
- Participate in the out of hours on call rota (additional pay)
- Complete any other appropriate duties and responsibilities as requested by the Registered Manager
- Assist with the implementation and set up of new packages, working closely with all involved in this process
- Assist with delivering effective staff inductions including meet and greets and shadow shifts
- Report any incidents/accidents or concerns in a timely manner through our internal reporting system
